Additional Information
1. Team Philosophy
Team members: Participants are greatly encouraged to register in “teams” composed of a minimally invasive surgeon and an interventional gastroenterologist (maximum number of persons per team is 2). As teamwork is considered an important aspect of the workshop, persons may take advantage of the reduced fee by joining forces. All participants are required to register for the workshop individually, whether as part of a team or not. When registering, persons who wish to participate as teams will be required to insert the name of their partner in the allocated field. Should the person assigned as partner not register for the workshop, the person registering as team member will be required to pay the individual registration fee. The outstanding balance will be payable by September 1, 2010.
2. Visas and Letters of Invitation
Visas are not required for visitors from most of European Countries. For all other countries, visas are issued on a reciprocal basis. It is advisable to contact the local Italian Embassy or Consulate. If required, a personal "letter of invitation" to facilitate the obtaining of visa or supporting funds for participation in the workshop will be issued by the conference secretariat (en2010@euro-notes.org). It should nevertheless be understood that this letter could not be considered as a commitment of the organisers to provide any financial support. This letter will be sent only to participants already registered.
3. Forms of Payment
When registering for the EURO-NOTES 2010 Workshop please arrange payment of the registration fee. All payments must be made in Euros. Participants may choose between the forms of payment below. We recommend payment via credit card.

4. Confirmation of Registration
Upon receipt of the registration form and the corresponding payment, each participant will receive a confirmation, which also serves as receipt. Please present this confirmation of participation at the registration counter when picking up your conference material.

6. Registration Deadline
- Early registration fee deadline for EAES/ESGE members: Monday, June 28, 2010 midnight CEST.
- Registration deadline: Monday, September 1, 2010 midnight CEST.
- After September 1, 2010, please reigster and pay on site.
7. Cancellation Policy
Notice of cancellation should be sent in writing only to the EURO-NOTES secretariat. In case of cancellation by July 15, 2010 the payment will be refunded, minus a handling charge of € 50. No refund will be made after July 15, 2010. Registrations will be confirmed if payment is received before September 1, 2010. Should the payment be received after this date, you will receive your confirmation at the conference counter. Please bring a copy of your bank remittance with you.
Changes to the Congress program: In the event of cancellations by speakers or other changes in the main conference program, no full or partial refund of the registration fee can be made.
